The safety of our participants and staff, and preventing the spread of COVID-19, remain our top priority.
We continue to follow the most stringent infection control and prevention practices to safeguard the well-being of our participants, families, and staff, as recommended by the Centers for Disease Control (CDC), and the California and County Departments of Public Health. Rapid testing for COVID-19 has been implemented for On Lok PACE participants and high-risk employees.
On Lok PACE services:
During the pandemic, we have shifted from PACE-center to home-based PACE services to keep our seniors safely sheltering in place. Participants continue to receive the medical care they need through their primary care provider and our network of specialists, and home-based PACE services are provided based on participant care plans, including:
Doctor home visits
Door-to-door transportation to medical appointment
Expanded home care by the On Lok PACE care team
Increased home-delivered meals services
Wellness and spiritual care calls
Telephone support groups
Personalized activity kits and newsletters delivered by Activities staff
Virtual YouTube activities @ Youtube.com/Onlok
For seniors and families in need of care, we continue to accept referrals and enrollments for seniors who qualify for our PACE services in San Francisco, Southern Alameda and Santa Clara Counties.
On Lok 30th Street Senior Center and senior services:
On Lok 30th Street Senior Center has been providing virtual activities and events to keep community members healthy and socially engaged.
We provide “takeout” lunches and meal delivery at On Lok 30th Street Senior Center.
We are piloting Always Active exercise and fall prevention classes as a virtual program.
