On Lok PACE (Program of All-Inclusive Care for the Elderly) is a full- service healthcare program that supports seniors to remain living at home as long as possible. Our team of healthcare professionals creates personalized care plans for each participant.

Our services include, but are not limited to, medical care; medication management and delivery; dental, vision, and hearing services; home care; transportation; home-delivered meals; and day center activities. When enrolled in On Lok PACE, your services must be received through On Lok contracted providers or you could be personally liable for costs incurred, unless it is an emergency or urgent situation.

We have a very diverse culture at On Lok PACE. We have staff who can speak languages, such as Chinese, Vietnamese, Spanish, English, Tagalog, and Hindi. For other languages, we offer interpretation services.

Yes, we have primary care providers (PCP) who will coordinate and take care of your medical needs and work with other medical professionals to make it easier for you. Our doctors and nurses are medically trained in senior care and very involved with each participant.

To qualify for On Lok PACE, you must be 55 years of age or older; live in San Francisco County, Santa Clara County, or the cities of Fremont, Newark, or Union City in Alameda County; need a nursing home level of care; and be able to live safely in the community.

When you contact us, we will gather some information to determine if you meet the basic criteria to join On Lok PACE. You will also connect with our nurse and care team who will gather further health information from you to determine if you meet the eligibility requirements for On Lok PACE. If approved, you will participate in an enrollment conference and can start receiving On Lok PACE services on the first day of the following month after you sign an enrollment agreement.

We want to ensure you receive the proper Medi-Cal benefits when you qualify. If you are unsure about whether you qualify or have Medi-Cal, we recommend that you contact your local social services office, or give us a call and we can help direct you to the appropriate office or resources.

On Lok PACE is a Medicare and Medi-Cal program.
If you are eligible for Medi-Cal, On Lok PACE services are covered at no additional cost. Medi-Cal beneficiaries who have a share of cost remain responsible for their individual share of cost.
If you do not qualify for Medi-Cal but you have Medicare, you will be charged a monthly premium to cover the long-term care portion of the On Lok PACE benefit and a premium for Medicare Part D drugs.
If you do not have Medicare or Medi-Cal, you can pay for On Lok PACE privately.
There are no deductibles, copayments, or coinsurance for any care and services approved by our team of healthcare professionals.
On Lok PACE is cost effective compared to a skilled nursing facility. In addition, you get to live comfortably in your own home. Please contact us to discuss your eligibility and individual situation.

No, participants must choose either IHSS or On Lok PACE. On Lok PACE is much more comprehensive as our program offers a wide range of medical and social services in addition to home care services.

Home care services may include help with grooming or showering; laundry; warming up food, medication reminders; and light housekeeping.

On Lok PACE is an alternative to a nursing home. Our mission is to help seniors remain living in their home as long as possible. Our personalized care plan supports that mission, and depending on each participant’s needs, care and services may be provided in the home, the community, or the PACE center.

On Lok PACE is a full-service coordinated healthcare plan. When a participant enrolls, our team of healthcare professionals creates personalized care plans for each participant. Therefore, each participant may receive different services depending on their needs. All participants, however, will receive medical services provided by On Lok PACE and/or our contracted providers.

There is no enrollment period to join On Lok PACE. You can start the enrollment process at any time upon meeting the PACE eligibility criteria. You can disenroll if you feel that the PACE program is no longer meeting your healthcare needs. Enrollments and voluntary disenrollments will be effective the first day of the following month of approval and/or notification.
