Support services for SFHP Medi-Cal members
On Lok has partnered with San Francisco Health Plan (SFHP) to provide SFHP Medi-Cal members with care management services to support their health and independence.
On Lok Enhanced Care Management (ECM) provides SFHP Medi-Cal members with a dedicated Care Manager who works with them and their healthcare team to assist in coordinating the care and resources they need to live independently.
The On Lok Enhanced Care Management Care Manager can help eligible SFHP Medi-Cal members:
Develop a personalized care plan and stay motivated to reach their health goals
Coordinate care and follow-up services
Learn ways to get and stay healthy
Update the care team on the participant's needs and preferences
Connect to community resources and programs to support the participant's health and independence, such as transportation, meals, and in-home services
On Lok Enhanced Care Management is a free benefit offered as part of the participant's Medi-Cal health plan. Medi-Cal services remain the same for On Lok Enhanced Care Management participants.
Who is eligible for On Lok Enhanced Care Management?
San Francisco Health Plan (SFHP) Medi-Cal member
Require some skilled nursing support
Need support with community resources, such as meals, personal care, medications, or transportation
Not currently receiving case management services
Able to live safely in the community with some assistance
