Bilingual case management and senior care coordination
The case management program partners with the Department of Disability and Aging Services (DAS) to help seniors or adults with disabilities access the care and services they need to live at home. Case managers can assist you with the authorization, arrangement, and coordination of services among providers and caregivers in the following areas.
Arranging and monitoring in-home support services
Paratransit services
Housing assistance
Assistance with Medi-Cal and Medicare counseling
Referrals to money management services, legal services or medical appointments
Advocacy and assistance with solving a variety of problems for homebound individuals
Our bilingual case managers (English and Spanish) may develop a care plan based on your needs, authorize and arrange for services, coordinate services among providers, monitor care, and help with discharge from care facilities.
To qualify you must be:
A resident of San Francisco
Age 60 or older or age 18-59 with a disability
At risk of premature institutionalization by meeting at least one of the following conditions:
Impairment in one or more areas of Activities of Daily Living (ADL) or in two or more Instrumental Activities of Daily Living (IADL)
Unable to manage personal affairs due to emotional and/or cognitive impairment; or
Impaired due to a significant event or circumstance occurred within the past 12 months
Not currently receiving duplicative case management services
