Helping seniors and people with disabilities live independently

Bilingual case management and senior care coordination

The case management program partners with the Department of Disability and Aging Services (DAS) to help seniors or adults with disabilities access the care and services they need to live at home. Case managers can assist you with the authorization, arrangement, and coordination of services among providers and caregivers in the following areas.

  • Paratransit services

  • Housing assistance

  • Assistance with Medi-Cal and Medicare counseling

  • Referrals to money management services, legal services or medical appointments

  • Advocacy and assistance with solving a variety of problems for homebound individuals

Our bilingual case managers (English and Spanish) may develop a care plan based on your needs, authorize and arrange for services, coordinate services among providers, monitor care, and help with discharge from care facilities.
