Faith Wells, M.D.

Dr. Faith Wells is Associate Medical Director at On Lok PACE Peralta Center. She is board certified in internal medicine.

Before joining On Lok, Dr. Wells was in private practice in Fremont. She has also worked as primary care physician for Palo Alto Medical Foundation and as a home care physician for Landmark Health.

Dr. Wells earned her medical degree with honors, graduating Alpha Omega Alpha and Phi Beta Kappa.

She is married and has two children. In her free time, she enjoys gardening and watching the SF Giants play baseball.
