With On Lok’s 50th anniversary right around the corner, we are reminded that our organization was created in response to the pressing needs of community seniors. Today, we are still responding to the needs of our community, providing the range of PACE services while ensuring your safety and preventing the spread of COVID-19.
We continue to follow the most stringent infection control, prevention practices, and testing guidelines to safeguard the well-being of our participants, families, and staff, as recommended by the Centers for Disease Control (CDC), and the California and County Departments of Public Health.
During the pandemic, we have shifted from PACE-center to home-based PACE services virtually overnight, to keep you safely sheltering in place. Your interdisciplinary care team is delivering the medical care your need through your primary care provider and our network of specialists. Our professional caregivers and home-care staff provide services for daily living in the safety and comfort of your home. Participants miss their favorite activities at their On Lok PACE center. During shelter-in-place, Activities staff keep them engaged and socially connected with a variety of virtual classes, exercise sessions, dance, music, games, and more. The Activities team reaches out with phone calls, care packages, and virtual events to let you know that you are not alone, but part of a caring community determined to get through this crisis together.
During COVID-19, medical and home-based PACE services staff are provided based on participant care plans, including:
Doctor home visits
Telehealth appointments
Door-to-door transportation to medical appointment
Expanded home care by the On Lok PACE care team
Increased home-delivered meals services
Wellness and spiritual care calls
Telephone support groups
Home-based exercises
Personalized activity kits and On Lok PACE center newsletters
